ONLINE STATEMENT ACCESS AGREEMENT
TRANSMITTAL OF ELECTRONIC STATEMENTS
By signing the “Request For Electronic Statement Delivery” you elect to receive your statements
electronically in the manner set forth by the University of Illinois Community Credit Union e-statement Procedures in question and answer format. You agree to accept your University of Illinois Community Credit Union periodic statements, and all disclosures related to your statements, electronically by a visual text to be displayed on a personal computer monitor. You request that the credit union transmit your statement to you electronically, and you agree to the terms and conditions set forth in this Agreement.
SECURITY PROCEDURES
You acknowledge that you will be able to retrieve your statements electronically through the use
of a personal identification number (PIN) which you will choose on your first log in. You acknowledge that, if you disclose your PIN to anyone else or if your PIN is lost or stolen, third parties may be able to access your credit union statements. You agree that the credit union will not be liable for any loss caused by the authorized or unauthorized use of your PIN by any third party to access your statements. You agree to keep your PIN in a place of safekeeping, and you agree that the security of your PIN will be your responsibility at all times. You agree to the security methods and techniques set out in the agreement. You agree that the credit union will not be liable for any loss arising out of your use of e-statement if the credit union observes the security methods presently set out in the agreement, or as amended from time to time. Further, you agree that the credit union will not be liable for any loss arising out of your use of e-statement under any circumstances if you fail to observe the security methods and the recommendations set forth in the procedures. You agree that it will be your responsibility to read current newsletters located within your e-statement layout.
ADDITIONAL PROVISIONS
You acknowledge and agree that, by requesting electronic statement delivery, you will not receive paper statements. You acknowledge and agree that, if the credit union in its judgment believes that the transmission of your statements electronically is no longer viable, or that any such transmission may give rise to security or operational concerns, the credit union may cease the electronic delivery of your statements and provide you with paper statements in lieu thereof. You acknowledge and agree that you have read and you understand the security procedures set out, that any e-mail address you provide to the credit union is accurate, that you will promptly notify the credit union of any change in your e-mail address in the manner prescribed in the security methods, that you have a computer that meets the minimum system requirements set out in the agreement, and that you have a printer or have access to a printer with the ability to download information in order to keep copies of your statements for your records.
SYSTEM REQUIREMENTS
The minimum requirements for e-statement are:
– Processor Speed: 486 or higher
– Modem Speed: 28.8 Kbps or higher
– Monitor Size: Any size, 800×600 resolution or higher
– Memory Size: 16MB or higher
– Adobe Acrobat Reader 5.0 or greater
Mobile Wallet Service Terms and Conditions
Terms and Conditions For Adding Your University of Illinois Community Credit Union (UICCU) Credit or Debit Card to Mobile Wallet Service (Retain for your records)
- Mobile Wallet Terms and Conditions These Terms and Conditions (the “Terms”) apply when you choose to add a UICCU credit card or debit card (“UICCU Card”) to a Mobile Wallet Service. In these Terms, “you”, “your”, “their”, and “my” refer to the cardholder of the UICCU Card, and “we,” “us,” “our,” and “UICCU” refer to the issuer of your UICCU Card. By selecting the check box entitled “I have read, and I understand and agree to the UICCU Mobile Wallet Service Terms and Conditions,” you agree to all terms, conditions, and notices contained or referenced in these Terms and you are providing your express consent.
- What is a Mobile Wallet Service? A Mobile Wallet Service (“Wallet”) allows you to add your UICCU Cards to an application using your Mobile Device. Your UICCU Card number is replaced with a digital number or token. Once added, you understand that you may use your Mobile Device to make payments only where the Wallet is accepted. UICCU is not a provider of the Wallet and we are not responsible for any failure or inability to perform a transaction using the Wallet. We are only responsible for supplying information securely to the Wallet provider to allow usage of the UICCU Card in the Wallet.
- Eligibility Active UICCU Card accounts that are in good standing are eligible to be added to a Wallet. If your UICCU Card or any underlying (UICCU) account becomes delinquent, is in a negative status or is otherwise maintained in an unsafe manner as determined by UICCU in its sole discretion, your UICCU Card may be removed by UICCU from the Wallet for continued use.
- Relationship to Other Agreements You agree that when you add your (UICCU) Card to a Wallet service, your UICCU Card and account will remain subject to the terms and conditions of all existing agreements with UICCU. You acknowledge that certain message and data rates may apply from your wireless service providers and/or wireless carriers which might impact your use of the Wallet. For example, your mobile service carrier or provider may impose data usage or text charges for your use of or interaction with the Wallet, including downloading the software, receiving or sending text messages, or other use of your mobile device when using the software or other products and services provided by the Wallet. You expressly agree that you are responsible for all such fees, limitations, and restrictions and that we may contact you via your mobile device for any purpose concerning your accounts at UICCU, including account servicing and collection purposes.
- Account Ownership/Accurate Information You represent that you are the legal owner of the account and other financial information which may be accessed via the Wallet. You represent and agree that all information you provide in connection with the Wallet is accurate, current, and complete, and that you have the right to provide such information to us for the purpose of operating the Wallet. You agree not to misrepresent your identity or your account information.
- Measures to maintain your privacy and security Once you have added your UICCU Card to a Wallet service, UICCU is responsible for securely transmitting your information to your Wallet service provider. Your information is only sent through secure channels. You agree that we may share your information with the Wallet provider, a payment network, and others in order to provide the services you have requested, to make information available to you about your UICCU Card transactions, and to improve our ability to offer these services. We do not control the privacy and security of your information that may be held by the Wallet provider and that is governed by the privacy policy given to you by the Wallet provider. You agree not to leave your Mobile Device unattended while logged into the Wallet and to log off immediately at the completion of each access by you. You agree not to provide your password or other access information to any other person. If you believe that someone may have unauthorized access to your Mobile Device, you agree to immediately cancel your access to the Wallet associated with the Mobile Device. You agree to provide us with immediate notice in the event you suspect fraud or any unauthorized access to any of your accounts. You agree to comply with all applicable laws, rules and regulations in connection with your UICCU Card.
You agree to take every precaution to ensure the safety, security and integrity of your account and transactions when using the Wallet.
We may in some cases make individually identifying information available only in the following circumstances: where ordered by a court or other legal body or where it is lawfully permitted or required; where we have agreed to provide information under contracts to vendors and partners to make products and services available to our members; or where the member has requested or consents to the disclosure of information. In those cases where we disclose information to outside vendors, we require that they use it for no purposes other than providing previously specified services to our members.
- Changes in these Terms and Conditions Except as otherwise required by law, UICCU may in its sole discretion change these terms, and modify or cancel the eligibility to use your UICCU Card with a Wallet service at any time, without notice. You cannot change these terms but you can terminate them by removing your UICCU Card(s) from the Wallet. UICCU reserves the right to refuse any transaction for any reason.
- Exclusion of Warranties; Limitation of Liability; Indemnification
YOU EXPRESSLY UNDERSTAND AND AGREE THAT YOUR USE OF A MOBILE WALLET SERVICE IS AT YOUR SOLE RISK. ANY MATERIAL DOWNLOADED OR OTHERWISE OBTAINED THROUGH THE USE OF THE WALLET IS OBTAINED AT YOUR OWN DISCRETION AND RISK, AND UICCU IS NOT RESPONSIBLE FOR ANY DAMAGE TO YOUR MOBILE DEVICE OR LOSS OF DATA THAT RESULTS FROM THE DOWNLOAD OF ANY SUCH MATERIAL, WHETHER DUE TO ANY COMPUTER VIRUS OR OTHERWISE. UICCU MAKES NO REPRESENTATION OR WARRANTY AS TO THE COMPLETENESS, ACCURACY, RELIABILITY, OR CURRENCY OF ANY INFORMATION OR DATA THAT YOU OBTAIN THROUGH THE USE OF A WALLET.

Shared Branching ID Check
How IDCheck works:
When you arrive at a participating Shared Branch location, you’ll scan a QR code with your phone and follow a few quick steps to verify your identity:
- Enter your Credit Union name & member info
- Take a picture of your government-issued ID, front and back
- Take a picture of yourself
- Get a one-time verification code to share with the teller
Once your ID and picture are uploaded, they’ll stay stored for future visits.
If you are not able to get a verification code this does not mean your transaction cannot be performed. The teller may be able to verify you by means other than the New IDCheck system.

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.
What this means for you:
When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents.
